PETITION TO CHANGE NAME COMES NOW, Petitioner, Yesenia Soto, by and through her undersigned attorney, and files this her Petition to Change Name and shows the Court the following: The Petitioners name is Yesenia Soto and she resides in Thomas County, Georgia. Therefore, jurisdiction and venue are proper in this Court. The Petitioner is of the age of majority, was born in New Jersey, see attached Exhibit A and is a resident of Thomas County, Georgia. Petitioner desires to change her name from Yesenia Soto to Yesenia Amadeo Ortiz. Petitioner desires to change her name aforementioned so that she can obtain a drivers license in the State of Georgia and to have it match the name on her residence. Petitioner does not intend to use the name change to fraudulently deprive anyone of any right under the law.
